Arachnoid Cyst Surgery: Dr. Jho's Endoscopic Fenestration of Cranial Arachnoid Cysts, Endoscopic Arachnoid Cyst Operation

Dr. Jho's Minimally Invasive Endoscopic Fenestration of Cranial Arachnoid Cysts

Professor & Chair,  Department of Neuroendoscopy
Jho Institute for Minimally Invasive Neurosurgery

As minimally invasive surgery, Dr. Jho performs endoscopic fenestration via a small incision for cerebral arachnoid cysts.  When an arachnoid cyst produces symptoms, a surgical treatment has to be considered.  Conventional surgery has been shunt placement into the cyst in order to drain the cystic fluid into the abdominal cavity.  Alternative treatment is endoscopic fenestration of the cyst so that the cystic fluid can drain into the cerebrospinal fluid pathway.

Figure 1. A patient with a left temporal arachnoid cyst developed headaches and right sided weakness. Preoperative MR scan, coronal view, shows an arachnoid cyst at the left temporal region (a black cavity) with brain shift to the right (A). Postoperative MR scan, coronal view, reveals reduction of the cyst fluid (black cavity) and return of the lateral ventricles to normal shape (two triangular black shadows at the upper center) (B). Intraoperative photographs show endoscopic view of the cystic wall (C) and wide fenestration of the cystic membrane (D). The operation was performed under the endoscopic visualization via a two-cm incision.
